Revivify by jass policy
Please be sure you have read our policy prior to booking an appointment.
• We require a valid Government Identification to keep on file.
• We require 48 hours’ cancellation or reschedule notice.
• Please be advised we take pictures of all our clients before and after treatment to follow up with each client’s progress.
• A deposit is required to guarantee your reservation and is non refundable. This deposit will be deducted from your final balance. The remaining balance is due after services are provided.
• Discounts do not apply to follow ups, touch ups and/or consultation appointments.
• We suggest you arrive 5-10 minutes prior to your scheduled appointment time. late arrivals may be asked to reschedule.
• Due to the nature of our detailed, artistic services We request that you not bring any guest with you to your appointment. This policy is strictly enforced!
• Revivify by Jass is an adult facility. No children allowed.
• We love having you as a client; however, we do reserve the right to refuse service to anyone.
